1.Exemplified excellent customer relations with clients, visitors, and other employees; shows courtesy, friendliness, and respect.
2.Made referrals, based on identified needs, to appropriate agencies/services or provides information and assists clients for self-referral.
3.Provided advocacy as needed to encourage participation from clients in resolving problems (such as transportation or language barriers).
4.Maintained files that documents services in accordance with agency policy.
5.Assisted patients in accomplishing tasks necessary to access services such as filling out appropriate forms, obtaining necessary documentation or authorizations.
6.Maintained professional standards and follows the treatment policies and procedures of the agency, as well as applicable federal, state, and local regulations.
7.Completed a daily schedule which documents every client visit, and time of visit.
8.Reported any unusual incidents to their supervisor. The Social Worker is a state mandated reporter, he/she will report to the state any suspected case of child/elder abuse or neglect as mandated by WAC.
9.Communicated clearly, both orally and in writing, with clients, providers, community professionals, and interdisciplinary team members. This includes maintaining records that are clear and concise regarding psychosocial and/or needs assessment, advocacy, referrals, and follow-up care. The documentation must reflect legal mandates for reporting abuse/neglect.
10.Developed working relationships with both, internal and external service system networks; network with resource providers serving this population to develop strategies to improve services delivery.
11.Demonstrated knowledge and skills necessary to provide care appropriate to the age of the patients served.
12.Identified and categorized each patient’s age-specific grouping of needs, such as those for infant, adolescent, or geriatric patients.
