Performs medical assessments on clients to include vital signs, oxygen saturation measurement, head to toe assessments, neurological examination, integumentary assessments or more focused assessments when warranted or ordered per primary care provider. Performs nursing patient assessments and treatments of clients with sudden minor or major illness, injury or changes in behavior in accordance with licensure. Provides wound care to major or minor wounds per primary care providers orders and as needed utilizing various wound care techniques. Responsible for contacting primary care provider of any changes in health condition and unusual behaviors for client. Provides or facilitates appropriate care for clients with chronic health problems such as diabetes, hypertension, COPD, seizure disorders, dementia, asthma, cognitive deficits and other mental and behavioral deficits in accordance with licensure. Coordinates medical, mental health, occupational health and physical therapy services with other departments within and without the facility to include Emergency Medical Services and medical specialists. Administers medications and various other treatments at ordered times. Collects blood and other laboratory specimens when ordered. Responsible for complete and accurate documentation in the individual healthcare record to include transcribing new physicians orders, notification of change in client health condition to all appropriate personnel to ensure proper continuity of care. Responsible for education of direct care staff when new conditions or problems arise. Performs routine, daily stock evaluations to include inventory of medical supplies, medications and chemical supplies. Responsible for notifying pharmacy of any medications needed. Provides patient centered care in a kind, respectful and caring manner to ensure clients rights are upheld and that they are dignified as wonderful human beings regardless of any disabilities one may have to endure.
