Care Coordinator

-Completes Evaluation and Assessment of needs, coordination of those needs and working with a team to ensure a successful recovery.
-Assesses Level of Care throughout the course of treatment as well as providing written and verbal follow up with referral sources and patients.
-Networks with referral sources and outside treatment providers and provides statistical feedback accordingly.
-Manages personal schedule, creates and keeps deadlines, and prioritizes job duties.

Gateway Rehab

Evaluation Therapist/ EAP Special Case Manager

Provides initial evaluations and Level of Care recommendations for Specials Cases including but not limited to: EAP, Employers, and Professional Monitoring Boards. This job includes preauthorization of services, case management of these cases and referral source communication throughout treatment up until discharge.

-Completes the initial Level of Care Evaluation.
-Diagnoses Substance Use Disorders based on Level of Care Evaluation.
-Recommends appropriate Level of Care based on criteria.
-Completes pre-certification process with appropriate payors.
-Provides comprehensive written and verbal follow up with referral source before, during and at discharge.
-Networks with referral sources and outside treatment providers.
-Communicates with therapy and other GRC/NKRC departments for case management purposes and to provide follow-up with the referral source.
-Maintains continual assessment of the unique needs of the referral source.
-Performs public speaking and provides training to other staff.
-Participates in group supervision and individual supervision.
-Works independently during after-hours to provide on-call admissions.
-Selected as one of Gateway’s Electronic Medical Record “Super Users."
These users learn the system inside and out and are the go-to people for any EHR questions other staff in your office may have. They’ll be responsible for providing tips and shortcuts to current users, as well as teaching new employees how to navigate the system. Super Users are identified by: ​

• Are able to learn new concepts quickly
• Are willing to help others learn the system
• Are natural leaders and well-respected by other staff
