I was a direct caregiver under the supervision of a charge nurse. I was responsible for the residents activities of daily living, such as bathing, feeding, dressing, positioning, transferring, taking vital signs, and recording of input and out put. Collection of specimens (urine, stool) as ordered. Provide skin care to prevent decubitus, carries out bowel and bladder management program and record results. Administer catheter care to resident that has Foley catheters. Assist in ambulation and record distances ambulated. Weigh residents, and report to charge nurse any fluctuations. Also making sure corrective devices are used appropriately; slings, positioning devices and hand rolls. Monitor chairs and bed check alarms. Answer call bell promptly. Report pertinent problems or significant changes in the residents condition. Maintain a clean, safe and healthy environment. Performs prescribed activity with residents and document on resident's accountability record. Attends scheduled educational in-services and complete all required documents in a timely basis.
