Executed ESG grant through the State Of New York for Jefferson County by providing Rapid Rehousing and Homeless Prevention services to individuals and families who are homeless or at risk of being homeless.
Maintain lines of communication with state agencies to clarify program regulation and procedures
Developed a case management and supportive service based housing program to address all areas of homelessness during housing crisis issues for clients.
Created professional relationships among other private, non-for-profit, and government agency to ensure clients were connected to appropriate services.
Participated actively in Points North Housing Coalition CoC to promote a tri-county wide commitment in order to end homelessness.
Distributed over 1,000 book bags to children in need.
Distributed over 1,300 warm weather items including coats, hats, gloves, scarves, boots, and snowsuits to individuals and children in need.
Leveraged over $43,000 in grant money for the Critical Needs Program.
Administered, along with Jefferson County Department of Social Services, Wheels for Work grant to help working families with children with purchasing vehicles to increase or keep employment as well as provided repairs to already owned vehicles.
Create policy and procedures to ensure fair advantage of all clients and ensure transitions among agencies are seamless
Execute a case management program to ensure housing stability goals are meant or exceeded
