•Manage a caseload of members pertaining to the elderly population, as well as people who have disabilities.
•Conduct Comprehensive Functional Needs Assessment with members in their home to assess their individual health needs.
•Encourage empowerment and self advocacy for members.
•Collaborate with member's primary care physician and home care agencies to better develop care plan based on the needs of the member.
•Connect member's to Behavioral Health providers in their area.
•Submit requests for durable medical equipment for member's through several agencies throughout Rhode Island.
•Determine the hours of personal home care and home health aide services member's need to continue living independently, and register each member for such services with different agencies throughout Rhode Island.
•Complete routine follow up phone calls with each member monthly.
•Provide adequate documentation for each member in computer database.
