Am the director of HIM for a well skilled health information management department. The current director had retired and her replacement is leaving.. Players in the revenue cycle were admissions/patient access, case management, charge capture, HIM, Un-billed management, business office, patient finance, compliance, IT, and physicians documentation in the medical record. Each player had to ensure their portion of the cycle was efficient. If just one part runs poorly or without collaboration with all the others, the whole flow will and does suffer. The departments mentioned above had to know each other's roles, not just their own. The Discharged not final billed occurs when the patient's bill has been held as a result of the system's edits such as lack/incorrect diagnosis/procedure coding, insurance verification, medical necessity, etc...Everyone bears some responsibility and everyone must provide a consistent effort to stay focused on their role and help the hospital maintain its bottom line goals. (HCPRO, AHIMA, TXHIMA, PRECYSE contributed to this description ) What really works for this hospital is that physicians perform the CDI function since the implementation of ICD-10.
