I had the opportunity to assist with the creation of procedures and processes for the newly opened Crisis Stabilization Unit within the H Group; located in Carterville IL. I was also responsible for the development of the treatment curriculum for the groups & individual sessions that the recovery specialists would facilitate daily. I was primarily responsible for conducting diagnostic assessments, treatment planning, brief therapy and clinical supervision to recovery specialists, RN’s and interns. I was accountable for reviewing and signing all other assessments, treatment plan & discharge plans. I conducted clinical treatment team meetings daily & discharge planning for guests outpatient care in the community. I provided supervision & direct treatment services to guests during their brief stay.
Providing consultation to primary care providers while promoting wellness to patients by providing brief intervention & therapy. Provided direct consultation to the primary care providers within the clinic; as well as assessments/screenings of healthcare needs and enhancing wellness through brief intervention & therapy with clients. I have had the opportunity to promote & provide coordination within the integrated health care project with management staff by working on several sub-committees; which include sustainability,communication & procedures within the program. Evaluated & analyzed overlap(s) with other programs within the agency to increase effectiveness towards clients and staff. Provided direct supervision & training to master's level social work students for their practicum experience.
Conducted Substance abuse, Mental Health & Co-Occuring Assessments and made appropriate levels of recommendations.
Worked with 18-21 year olds diagnosed with a behavioral health condition who are integrating into the community.
Provided around the clock support, mentoring and education to build the skills necessary to integrate into the community.
