Assist consumers by providing sufficient information and empowering them to make informed choices. Enable consumers to have access to options counseling, person centered support and, as needed, completion of applications for benefit programs. Utilize a Universal Intake Form (UIF) to determine financial and programmatic eligibility for public programs, track eligibility and provide appropriate follow-up to determine verification of services received and/or if the consumer requires additional guidance or service has been met. Assist discharge planners from hospitals, rehabilitation centers and nursing homes to transition residents back into the community. ADRC reimbursement through Medicaid is allowed for time spent providing client driven ADRC services. Responsible for conducting daily and monthly reports that are sent to the Alabama Department of Senior Services.
