o Act as a project manager by evaluating risk/appropriateness of potential kidney recipient; determine if potential/current risk factors will negatively impact future outcomes; and measure progress of patient in addressing potential barriers to organ transplant.
o Assist patients in addressing psychosocial issues identified, which include referrals to mental health
treatment, conflict resolution, drug/alcohol treatment, and social services.
o Manage a population of 200 post-transplant kidney recipients, in addition to evaluating an
average of 6 new potential organ transplant recipients per week.
o Collaborate with dialysis centers and patient service providers for continuity of care
o Coordinate appropriate services and resources.
