St Monica's is a 12 bed Community hospital with one specialised Palliative care suite. For the majority of shifts, I was the sole nurse on duty, therefore in charge of the hospital, including non-medical staff. The hospital specialises in aged care, rehabilitation and palliative care. My main roles and responsibilities included medication administration (including the care of syringe drivers), basic nursing duties involving patient care, wound care, working within a multi-disciplinary team to ensure effective admission and discharge planning, clinical education and supervision of junior staff members and student nurses, effective communication in a caring manner with patients and their families, administrative duties including writing and reassessment of care plans and ensuring that legislation guidelines and local policies were adhered to.
Oaktrees is a 38 bed nursing home for elderly people requiring 24hr nursing care. My duties included supervision of a care team consisting normally of 5 Care Assistants, ensuring all personal cares for the residents were completed to a high standard, ensuring privacy and dignity were maintained at all times. Documentation including regular updates of care plans, and involvement with other members of the multidisciplinary team, including GP's, Psychiatrists and Care managers was pertinent to the role. Effective communication skills were also required to meet with family members, which was crucial in my role due to the environment I worked in. Administration of medication, catheter care, wound care and personal cares were also involved in his role.
In this role I was Nurse in Charge on night duty, of a 30 bed home for people with varying physical disabilities, including cerebral palsy, multiple sclerosis, post CVA, post trauma head injuries and spinal injuries. General nursing duties included personal cares, which were paramount, alongside effective communication, which was individual for,each service user due to their varying disabilities. Each shift also included updating of care plans and daily records, medication administration and catheter care. I was in charge of 3 Care staff per shift; mentoring and supervising role was involved on every shift to ensure all service users care needs we're met.
