Contacted providers to obtain information on all patients with an abnormal finding focusing on the patients that are non-complaint.
Conducted regular Case Manager Site visits to monitor compliance and barriers.
Ensure that providers will schedule and perform diagnostic services and complete follow up form in a timely manner.
Assisted providers with recruitment through health fairs and other events, which increased patient enrollment.
Assisted patients that are newly diagnosed with the process of screening for the Medicaid Treatment Act and/or other resources for treatment and diagnostic services.
Provided translation and interpretation services for non-English speaking patients.
Translated pamphlets, consent forms, letters, educational flyers and other documents for the use of the program.
