Special Assistance/In Home (SA/IH) program provides additional income to adults who meet the criteria. These adults have been determined by a doctor to need domiciliary (e.g., adult care home, assisted living, family care home, rest home) level of care. Through this program these adults would be able to remain in their homes provided they are safe and the additional monies are sufficient to meet the needs. The SA/IH payment can be used to pay for services that would then stabilize the adults’ home environment. The program can be used to pay for: needed household repairs, utility expenses, respite, groceries, medical supplies, clothing, rent/mortgage, transportation, etc.

Duties and Responsibilities
• Monitors, coordinates and assist Community Alternative Program/DA clients in the home to ensure their health, safety and well-being.
• Verifies Medicare and Medicaid coverage.
• Verifies all services rendered to the client such as IHA & Nurse visits, durable medical equipment, diabetic testing supplies, etc.
• Reviews and coordinates with the RN her assessment of the client’s physical condition and what ADLs and IADLs the client is able to perform independently.
• Reviews and verifies family support as required by CAP/DA.
• Determines the type and amount of assistance required to keep the client at home in a safe environment and prevent the client from being admitted to a facility of care; develops a Plan of Care.
• Sends approval notifications to all providers once DSS approves notice of benefits.
• Contacts the client monthly via telephone, and quarterly home visits to assess the client; notify service providers.
• Verifies billing information from Medicaid and makes any modifications if needed; prepares in house billing for in home services.
/conducts Continued Needs Review Assessments and prepares paperwork for agency and service providers to continue services.
• Conducts Change of Status: re-admits for clients in crisis/ substantial changes.
• Coordinates and maintains a good working relationship with service providers and health care entities to obtain needed approval and information to coordinate services for clients.

Additional Job Duties:
Back up Intake and Outreach Social Worker for energy assistance and emergency assistance programs.

Back up Adult Protective Services Social Worker.

Safety Attendant/ Nursing Assistant

assisting the management of psychiatric persons served to include behavioral and clinical needs.
Works closely with the staff RN to facilitate the optimum habilitation plan.
Documents behavior changes of the psychiatric person during a 12 hour period to report off to fellow clinical staff and supervisors.
Experience working in settings with limited daily supervision.
Extensive practicing of HIPPA laws and ability to handle confidential information/ materials with discretion.
