•Monitor daily orders for Physicians in acute rehab to ensure continuity of care for the patient identifying new infections, falls, skin integrity, change of conditions, behaviors, and medication changes. Review all new admissions including reason for admission, co -morbidities, proper disease and medication management.
•Communicate with patients and families discussing goals, any change in condition, risk and benefits based on care planned goals.
•Daily case management of patient including follow up on all clinical consultations, new orders, follow up appointments and insurance compliance including Medicare coverage.
•Management Risk Review with all necessary disciplines to discuss residents at high risk who trigger for increase with skin wounds, falls, weight loss or any other set back that could compromise a positive outcome.
•Meet with facility Medical Director to update on residents. Participate with Medical
Director and associated care team members in weekly Grand Rounds.
•Attend staff meetings to monitor all Medicare updates, Coumadin dose changes, and initiate all reportable incidents in accordance with Wisconsin State Regulations. Weekly marketing strategy meetings, psychotropic review for medication reduction based on pharmacy recommendations. Monthly Quality Assurance Process Initiative to track trends analyzes management reports and process improvements.
•Communicates all updates including new admissions with front line staff, room readiness, patient dignity, and the importance of exceptional care. Reinforce the importance of listening to customers and meeting their needs on a daily basis.
