Crossroads Hospice of Cleveland

State Tested Nurse Assistant

I travel to different facilities and homes of patients who have come onto our services. For home patients I help assist families in the care of their loved ones. I help educate families in ways to help reposition patients while in bed, changing bed linen while patient is in bed, and ways to transfer using proper body mechanics. For patients in facilities we help staff with feeding patients if needed, give showers, and help in morning care for patients. Any declines, mor changes in condition we see with our patients we are to report to our charge nurse.

I worked as an aide on the 300 hall in the facility for first shift which was 7-3. I would receive verbal report from the night shift staff. The patients who were already dressed and ready for the day I would help bring down to the dining room as breakfast would start serving at 7:30 or 7:40. Care consisted of helping patients with morning care, peri-care, mouth, skin, and nail care. At times floor nurses would ask us to take monthly weights on our residents or help take needed vitals. If I noticed any change in condition or a change in a patient's mental status I was to report my findings to the nurse. At the end of each shift either myself, or one of my team members were to chart on each patient for our shift. Working on this unit there were usually 3 aides to a floor so it took team effort to help the care of the residents go smoothly for the shift.

I worked on the second floor as a nurse aide for second shift. I would receive verbal report from the previous shift of my section where I would be responsible for taking care of 10-12 patients. My duty was to pass out linen for care and to prioritize the care of patients depending on showers and what patients wanted to go to bed earlier than others. I would assist in feeding during dinner and set up for all patients on the second floor. I assisted patients I evening care which sometimes consisted of toilet patients, peri-care, skin and mouth care. Also helping patients get dressed for the evening. Depending on the needs of patients sometimes another aide would help me with transfers or a lift such as stand up or hoyer lift was needed.

I was hired as state tested nurse assistant. I worked on the long term care unit. I assisted patients in bathing, changed bed linen and made beds, assisted patients in feeding with different types of diets such as mechanical soft, regular, or purée diets. Assisted patients in transferring whether it was limited use of body mechanics or the use of different lifts. Helped patients in dressing for the day or evening depending on the shift. Provided companionship to patients who were able to make needs known, and share stories.
