Transitional Coordinator for Money Follows the Person (MFP) Program

-Utilize person centered approaches to identify barriers for individuals in skilled nursing facilities to transition back into the community to improve their quality of life
-Collaborate with State of Connecticut Department of Social Services and Mental Health and Addiction Services to initiate MFP waiver services
-Direct initial Quality of Life (QOL) survey and work with University of Connecticut's Center on Aging for follow-up data collection
