Intensive Case Manager

Coordinates, links, monitors and assesses mental health services for youth with significant and persistent mental health needs. Develops needs assessments and service goals with client, monitors progress towards goals and assesses mental health throughout service delivery. Works closely with parents to help eliminate barriers to client success and foster positive change. Participates in school meetings and often acts as a bridge between school and home. Works with client to identify and utilize natural supports and resources in their community. Develops safety plans with client and parents and supports objectives of therapeutic treatment goals. Helps client and family access crisis resources in the community such as crisis ER, hospitals, respite beds, Parsons Mobile Crisis, DSS, and foster care. Helps parents access their own mental health care when needed and participate in parent support groups.

MICA Coordinator

Provided outreach and coordination services to adult residents of Rensselaer County seeking treatment for co occurring mental health and substance abuse issues. Met with potential clients in their homes, at community shelters, the Roarke Center, Bethany House, and various other sites throughout the county. Linked clients to such providers as RCDMH, Hudson Mohawk, Conifer Park, Samaritan Hospital, Unity House, CEO, and other providers including housing programs. Held information and outreach sessions at community shelters establishing important connections with chronic and high service consumers.

Provided substance abuse prevention education services to students grades K-9 at Rensselaer City School District. Delivered classroom-size, curriculum-based prevention education lessons. Provided individual and group counseling, responded to student crisis, developed safety plans, served on IST and CSE committees, referral source for families wanting to access treatment services in their community. Arranged a district-wide Internet safety awareness training for parents and students. Worked closely with administration to provide substance abuse assessments and intervention planning. Ran Strengthening Families, a science-based parenting workshop that included youth participation. Developed the first student mentoring program at the district with the school social worker.

School Guidance Counselor

Provided individual, group, and crisis counseling for students K-6; extensive prevention curriculum in the classrooms with lessons covering topics such as bullying, identifying and expressing feelings, conflict resolution, and personal safety. Small group lessons on topics such as incarcerated parents, anger management, divorce, friendship club, study skill development, stress reduction, and social skill building. Managed the Instructional Support Team, connected students with such character building programs as Girls On The Run, volunteering, summer camp, and clubs. Oversaw grant-funded mental health program and the child associate that provided the in-school service to students. Worked closely with all school personnel to develop strong guidance involvement in student academic, social, and emotional success.
