Family Development Housing Coordinator
-Primary responsibility EBCH housing program – case management, intake and property issues
-Coordinates Rental Assistance Program supervision of Rental Assistance and AmeriCorps staff
-Lead Education Component--educate families with lead poisoned children and provide trainings and outreach, working w/RI Housing
-Interviews and evaluates homeless families for appropriate intake into supportive housing or for external referral, based on defined criteria; or when a unit is available. Maintains a client waiting list. -Works with these families, including low-income, homeless families, by providing Case Management and developing an action plan with each family.
-Creates a “Plan of Action” with each client to stabilize the family and to assist them to function as independently as possible. This plan analyzes the effectiveness of service to each client and insures that services mesh and that proper follow up will occur. This plan is updated at least quarterly. Meets with clients weekly in the office or home. Serves as a source of support to clients throughout the program. Meets with clients individually as requested or needed. Completes a client discharge narrative at the end of program stating completion and outlining progress on outcomes and success.
Maintain detailed case notes, goals achieved and other benchmarks of progress, and does a monthly caseload report. Maintains a current EBCH house policy forms and procedures including client check list, monthly property maintenance check list, exit plan, and house check list for each family. Maintains updated statistical data for grant reporting.
Conducts and completes a monthly property maintenance checklist with each unit/client.
This assures the internal and external upkeep of property; completes an exit plan with each family as they leave the program; and monitors families that have successfully completed the program.
Coordinates policies and procedures for housing program under the direction of the Housing Director
